Assess urgently for testicular torsion, incarcerated hernia, epididymo-orchitis, hydrocele, varicocele, malignancy, and other acute scrotal pathology.

Red flags
- Sudden painful swelling
- severe unilateral swelling with nausea or vomiting
- high-riding or abnormal lie of testis
- irreducible groin/scrotal swelling
First actions / assessment
- ABCDE first if unwell
- record full vitals, pain severity, and whether swelling is acute or progressive
- clarify unilateral versus bilateral swelling, pain versus painless enlargement, onset, progression, trauma, fever, dysuria, urethral discharge, abdominal or groin pain, vomiting, sexual history where relevant, and prior similar episodes
