Assess urgently for testicular torsion, epididymo-orchitis, incarcerated hernia, trauma, referred pain, and other acute scrotal emergencies.

Red flags
- Sudden severe unilateral scrotal pain
- high-riding or abnormal lie of testis
- nausea or vomiting with acute scrotal pain
- absent cremasteric reflex if assessed
First actions / assessment
- ABCDE first if unwell
- record full vitals, pain severity, and assess for vomiting, systemic symptoms, and trauma
- clarify onset, exact location, unilateral versus bilateral symptoms, progression, swelling, dysuria, urethral discharge, fever, abdominal or flank pain, trauma, sexual history where relevant, and prior similar episodes
