Assess urgently for cellulitis, septic arthritis, necrotizing fasciitis, DVT, acute limb ischaemia, and severe soft tissue infection.

Red flags
- Sepsis physiology or shock
- rapidly progressive redness/swelling
- pain out of proportion to findings
- skin necrosis, bullae, or crepitus
First actions / assessment
- ABCDE first if unwell
- record full vitals, perfusion, and pain severity
- clarify onset, progression, trauma, bite, wound, diabetes, immunosuppression, fever, rigors, inability to bear weight, calf swelling, chest symptoms, and whether pain is localized to skin, soft tissue, joint, or whole limb
