Assess urgently for unstable arrhythmia, AF/flutter, SVT, VT, stimulant/drug causes, electrolyte disturbance, and cardiac syncope.

- ABCDE first
- if current palpitations are present, obtain a 12-lead ECG immediately if possible including rhythm strip
