Assess urgently for cervical cord compression/myelopathy, meningitis, vascular dissection, fracture/instability, infection, and serious neurological causes.

- Severe or sudden neck pain with neurological deficit
- clumsy or weak hands
- gait disturbance or inability to walk
- bilateral arm or leg symptoms
First actions / assessment
- ABCDE first if unwell
- record full vitals, pain severity, neurological status, and capillary glucose if indicated
- clarify onset, duration, trauma history, headache, fever, photophobia, radiation to arm, numbness, weakness, hand clumsiness, balance problems, bladder/bowel symptoms, weight loss, cancer history, infection risk, anticoagulant use, and vascular symptoms
