Assess urgently for haemodynamic instability, major blood loss, variceal bleeding, and causes requiring immediate resuscitation and endoscopy.

- Shock or haemodynamic instability
- haematemesis or coffee-ground vomiting with ongoing bleeding
- melaena with syncope/presyncope
- persistent tachycardia or hypotension
First actions / assessment
- ABCDE first
- assess whether the patient is unstable and requires immediate resuscitation
- record full vitals including postural compromise if safe and relevant
