Assess urgently for heart failure, nephrotic syndrome, renal failure, liver disease, severe hypoalbuminaemia, myxoedema, and fluid-overload states with respiratory compromise.

First actions / assessment
- ABCDE first if unwell
- record full vitals, oxygen saturation, weight if feasible, urine output where possible, and assess perfusion and mental status
- clarify duration and progression of swelling, whether generalized or mainly dependent, associated breathlessness, orthopnoea, reduced urine output, frothy urine, jaundice, abdominal distension, weight gain, medication triggers, pregnancy history where relevant, thyroid symptoms, and systemic symptoms
