- Haemodynamic instability or shock
- positive pregnancy test with pain and/or bleeding
- syncope or presyncope
- severe unilateral pelvic pain
First actions / assessment
- ABCDE first if unwell
- record full vitals, perfusion, pain severity, and pregnancy status immediately
- obtain focused history including onset, duration, site, unilateral versus bilateral pain, relation to menses, vaginal bleeding/discharge, pregnancy possibility, last menstrual period, syncope, shoulder-tip pain, urinary symptoms, bowel symptoms, fever, STI risk, fertility treatment, and prior ectopic/ovarian cyst history
