Red flags
- Sudden onset diplopia
- diplopia with focal neurological deficit
- ptosis or anisocoria
- painful eye movements
First actions / assessment
- ABCDE first if systemically unwell
- clarify whether the diplopia is monocular or binocular by asking if it persists when either eye is closed
- record full vitals and capillary glucose if indicated
