Assess urgently for ACS, PE, aortic dissection, pneumothorax, pericardial disease, and other life-threatening causes.

Red flags
- Ongoing or severe central chest pain
- pain lasting more than 15 minutes
- pain radiating to arm, jaw, shoulder, or back
- associated diaphoresis, nausea, vomiting, or dyspnoea
First actions / assessment
- ABCDE first
- record full vitals and repeat frequently if high risk
- obtain 12-lead ECG as early as possible and compare with old ECG if available
